Used to open a new non-IRA Sit Mutual Fund account for individuals, joint tenants, UTMA/UGMA, trusts, estates, or self-directed retirement plans. There's also a separate Legal Entity version for corporations, partnerships, LLCs, etc. Additional documentation (e.g., trust pages, corporate resolutions) is often required.
Affidavit of Domicile

A notarized sworn statement verifying a deceased person's state of residence at the time of death. Used by personal representatives, surviving joint owners, or beneficiaries when handling estate-related transfers. Death certificates are not a substitute.
Bank Authorization

Adds a bank or credit union to your account for electronic funds transfers (ACH). Allows purchases to be funded directly from your bank or redemption proceeds to be sent there (no fee).
Change of Account Information

Checkwriting

Cost Basis Election

Coverdell Application
